New patients and children are welcome at Sunbeam Dental.
Patient forms
Download our patient form, fill it out and bring it with you on your first visit.
The form includes your registration, your medical and dental history, the consent to your first examination, the authorization to submit insurance claims, and the acknowledgement of our privacy notice.
Insurance
Our office accepts most PPO insurance plans and any dental plan that provides out-of-network benefits, including Aetna, Delta, Metlife, Horizon, Blue Cross/Blue Shield, Cigna, UFT, State of New York Employee Benefit Fund, Unions, Locals, Unisys, Americhoice, and more. We are also providers for various PPO plans and discount plans.
In all, over 220 insurance plans are accepted, including NJ Medicaid.
If you have dental insurance, please bring your insurance card for photocopying. This allows us to file a claim on your behalf.

Payment plans are available. We accept Visa, Mastercard, American Express and Discover, as well as debit cards and contactless mobile payments.
